How D-mannose works — and why it’s different to cranberry alone

Most people have only heard of cranberry juice for UTIs. D-mannose is the scientific mechanism behind why it partially works — isolated and concentrated.

The overwhelming majority of urinary tract infections are caused by E. coli bacteria adhering to the uroepithelial cells lining the urinary tract. Adhesion is the first step — without it, bacteria cannot establish infection.

D-mannose is a monosaccharide that competitively binds to the FimH adhesion proteins on E. coli that the bacteria use to attach to urinary tract cells. D-mannose saturates these binding sites, so the bacteria attach to D-mannose instead — and are then flushed out in the next urination. No adhesion. No infection.

Cranberry contains proanthocyanidins (PACs) which have a similar anti-adhesion mechanism against other E. coli strains. The combination of D-mannose and cranberry PACs addresses a broader range of bacterial adhesion pathways than either alone.

Common questions

Can D-mannose treat an active UTI or just prevent them?

Research shows D-mannose is effective for both prevention and treatment of E. coli UTIs. For an active infection, higher doses (2g, 3× daily) are studied. For prevention, 2g daily maintenance is the typical protocol. If you have an active infection with symptoms such as fever, consult your GP — antibiotics may be necessary.

Is it safe for diabetics?

D-mannose is a simple sugar but is metabolised differently to glucose and has minimal effect on blood sugar at supplemental doses. However, if you have diabetes or insulin resistance, consult your GP before use as individual responses vary.
